Our Foundation

You can now make a donation to the Aylmer Health Coop (AHC) through the Fondation des Services de Santé d’Aylmer (FSSA). Tax receipts will be issued.

Donations received by the AHC and for the AHC (specified donations) will be applied to the Coop’s various projects, e.g., building improvements, purchase of medical equipment, implementation of various programs, and so on.

To make a donation, you may fill this out the online form, then print it and forward it to the Aylmer Health Coop or print this form , fill it out and send it to the Aylmer Health Coop.  A tax receipt will be issued and sent to you.  Thank you!

Federal Government public servants and retirees

Federal Government public servants and retirees are invited to make their annual Government of Canada Workplace Charitable Campaign (GCWCC) donation through section «C» of their form by indicating the name La Fondation des services de santé d’Aylmer and writing down the foundation registration number   888791662RR0001. Look at this form.
